Welia Health Recertified as Level 3 Health Care Home

Welia Health is proud to announce that its primary care clinics in Hinckley, Mora, and Pine City have been recertified by the Minnesota Department of Health as Level 3 Health Care Homes — the highest level of recognition in the state’s certification program.

First certified in 2019, Welia Health met rigorous standards for providing patient- and family-centered care and will continue working to improve the quality, experience, and value of care for the communities it serves.

What it means to be a Health Care Home

Health Care Homes (HCH) deliver complete care ranging from disease prevention to the management of complex and chronic conditions. This team-based approach makes it easier for patients to communicate with and partner in their care, creating a true “home” for health services.

As a certified Health Care Home, Welia Health coordinates care with specialists and community services to ensure patients get the resources they need to maintain and improve their health. This includes:

• Expanded access to appointments
• 24/7 access to a clinic representative
• Proactive care planning
• Designated patient care coordinators
• Ongoing quality improvement initiatives

Why Level 3 certification matters

Achieving Level 3 certification means that Welia Health not only meets foundational and Level 2 requirements — which include efforts to reduce health disparities, enhance equity, and strengthen community partnerships — but also goes further to integrate care with broader community health improvement efforts.

This includes contributing to community health needs assessments, participating in population health improvement strategies, and actively engaging in implementing and monitoring progress on community health initiatives.
