Before your appointment

If you are covered by health insurance
  • Confirm your health plan is accepted by Welia Health and in-network. View accepted health plans.
  • Ask your health plan if specific services are covered or require a referral or pre-authorization.
  • Review questions about copays, deductibles and out-of-pocket costs with your health plan.
  • Complete the Assignment of Benefits and Consent for Use and Release of Health Information. Welia Health requires these forms to be on file in order to bill your insurance company. Print, complete and bring to your appointment or if you prefer, fill out at check-in the day of your appointment.
If you are NOT covered by insurance
  • Use the tools below to better understand hospital and clinic charges.
  • Get an estimate for any of our top 300 hospital services.

Understanding costs and estimates

Welia Health has several tools to help you better understand the cost of care before receiving service. Review publicly available lists of charges or generate a personalized estimate online.

Review hospital and clinic charges

In accordance with state and federal law, all hospitals are required to post all standard charges publicly. View Welia Health’s charge master. Updated January 1, 2024.

Get a price estimate

Welia Health is committed to price transparency related to the cost of care. Log into MyChart to generate an estimate of our top 300 shoppable hospital services.

When you receive emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing. Learn more about Welia Health’s no surprise billing policy.
