For your first visit, please bring a list of your current medications (including over-the-counter drugs and supplements), your medical history (including past surgeries and chronic conditions), your insurance information, and any questions you would like to ask. This helps us make the most of our time and build a care plan tailored to you.

We offer well-child visits, annual wellness exams, immunizations, screenings (including blood pressure, cholesterol, colorectal cancer, breast cancer, cervical cancer, prostate cancer, osteoporosis, diabetes, and others), and guidance on healthy living.
